Publication pending OVERARCHING QUESTION For women undergoing vaginal gynecological surgery for benign indications, how do specific contemporary tools or technologies facilitate or enable intraoperative performance of vaginal surgical procedures? POPULATION: Women undergoing vaginal gynecological surgery for any benign indication ● Include: o Total or subtotal vaginal hysterectomy, salpingectomy, oophorectomy, ovarian cystectomy, other adnexal surgery, myomectomy, pelvic reconstruction (apical suspension – USLS/SSLF/sacrocolpopexy, colporrhaphy), fistula repair o Vaginal surgical procedures that can also be performed by an alternate route (abdominal, laparoscopic, or robotic) ● Exclude: o LAVH, LH, TLH, exclusively laparoscopic/robotic/abdominal approaches o Vulvar, Non-GYN (colorectal, urology, etc) and obstetric (e.g. D&C, laceration repair) surgery o Malignancy o Age<18 years o Procedures that are exclusively vaginal that cannot be performed by another approach (exclude midurethral sling, vaginal mesh procedures, CKC, LEEP, vaginal cyst excision, hysteroscopy, GYN D&C, urethral diverticulectomy) o Office procedures (e.g.colposcopy) o Transgender procedures/Neovagina creation o Reproductive Endocrinology (REI) procedures INTERVENTION: A contemporary tool or technology that facilitates or enables intraoperative performance of a gynecologic vaginal surgical procedure ● Include: o Telescopic cameras (Video Telescopic Operating Monitor - VITOM) o Transvaginal Natural Orifice Transluminal Endoscopic Surgery (vNOTES) o Retractors (vaginal bookwalter, alexis, Martin’s arm, Lonestar, lighted hand-held) o Vessel sealing devices (update to 2016 paper) o Vaginal Robot (Hominis) o Capio/Anchorsure/EnPlace o Endoloops o Lighted suction o Any interventions that aid surgeon, assistants, learners ● Exclude: o Older technologies (cold clamps/instruments, needle drivers, hemostatic agents) – focus on contemporary tools/technologies o Stapling devices o Surgical technique modifications that do not specifically involve an enabling technology (e.g. modified Shull USLS, modified McCalls, suture or technique modifications) COMPARATOR: ● Another alternative tool or technology, including no intervention, that aims to accomplish the same goal at a similar point in the procedure OUTCOMES: Intraoperative ● EBL ● Operative Time ● Complications (GU, bowel, vascular, vaginal, vulvar, other injury) ● Transfusion ● Surgeon/Assistant Ergonomic Strain ● Procedure Success/Conversion to another route ● Education (Learner satisfaction, improved visualization) Postoperative • Hemoglobin • Transfusion • Reoperation/Return to OR • Length of Hospital Stay/same day discharge • Complications/Adverse events – Delayed complications, fistula, hematoma • Infection – UTI – Surgical wound – Pelvic • Postoperative Pain – Subjective (VAS) – Objective (Narcotic Use) • PONV • GI Function (constipation) • Dyspareunia • QoL/Patient satisfaction • Cost • Fertility/future pregnancy STUDY DESIGN: ● Include o Comparative studies: ▪ RCTs, Case-control, Prospective or retrospective observational comparative studies ▪ Vaginal surgery arms of comparative studies as single group cohort (e.g. study on vNOTES vs TLH – include vNOTES arm if n>10 of a particular procedure - e.g. 10 hysterectomies or 10 salpingectomies, not 4 hysterectomies and 6 salpingectomies) o Systematic Reviews o Single Group studies: ▪ Prospective or retrospective, n>10 of a particular procedure ▪ Video case series with n>10 of a particular procedure and any outcome of interest o Any language ● Exclude o Abstracts, meeting presentations o Narrative reviews o Animal/non-human studies o Cadaver studies o Radiologic studies that do not involve vaginal surgery o Case reports
