| Botulinum toxin type A piriformis injection | 300 units 3 ml intramuscular injection to 4 piriformis muscle sites (the most medial injection was made just lateral to the greater sciatic foramen along the line linking its midpoint to the greater trochanter) including the myoneural junctions of the S1 to S2 nerve fibers that innervate the piriformis muscle; placement with electromyographic guidance |
